Are you passionate about making a difference in the lives of children? Therapedia, LLC, a growing pediatric therapy practice, is seeking a Patient Care Coordinator to join our team.

About the Role:

The Patient Care Coordinator plays a vital role in enhancing the patient experience and fostering relationships within the community.
In this role, you will:

• Assist families in establishing therapy services by educating them on the referral process, verifying benefits and costs, obtaining authorizations and scheduling initial evaluations.
• Serve as a liaison between patients, families, and referral sources to ensure smooth and efficient processes.
• Promote community engagement by coordinating educational opportunities for clinicians and referral sources.
• Provide exceptional customer service while maintaining professionalism and accuracy in all interactions.

Essential Functions:

• Develop and implement strategies to increase engagement with referral sources.
• Conduct patient outreach via phone, email, text, and video calls to answer questions and facilitate onboarding.
• Maintain accurate and confidential client records in compliance with HIPAA standards.
• Build and sustain strong relationships with healthcare providers and community organizations.
• Collaborate with clinical and front office teams to ensure a seamless patient experience.
• Gain a thorough understanding of Therapedia’s services and communicate them effectively.

Qualifications:

• High school diploma or equivalent required.
• Ability to thrive in a fast-paced environment.
• Exceptional communication and customer service skills.
• Detail-oriented, self-motivated, and a natural problem-solver.
• Professional appearance and demeanor.
• Proficient in using computers and various communication tools.
