Responsibilities:

• Conduct and coordinate comprehensive MDS assessments in compliance with state and federal regulations.
• Develop and update individualized care plans based on assessment findings and interdisciplinary team input.
• Ensure accurate and timely documentation to support optimal reimbursement and regulatory compliance.
• Collaborate with nursing staff, therapists, and other healthcare professionals to deliver comprehensive care.
• Participate in care plan meetings and contribute to continuous improvement initiatives within the facility.
• Monitor and audit MDS processes to ensure quality outcomes and compliance with all relevant regulations.

Qualifications:

• Current California LVN license.
• Proven experience as an MDS Coordinator in a skilled nursing or long-term care setting.
• Strong understanding of MDS 3.0, care planning, and regulatory requirements.
• Excellent organizational skills and attention to detail.
• Strong communication and teamwork skills, with the ability to work collaboratively with a multidisciplinary team.
